What to Expect Upon Your First Visit
How to Get Started
Please obtain previous medical records if transferring from another provider. This will make your visit with the provider more efficient and allow more time for clinical evaluation. If you wish for a provider at Women’s & Maternity Care Specialists to review a test or imaging result from the hospital or testing facility you must provide those documents. You may authorize the release of your previous records to us by completing the form here.
The Patient Portal is a great way to interact with our office and your important clinical information and results. Upon making an appointment, you will be given an invitation to use the portal through an email sent to you. Using this email invitation, you will be able to log in to the Portal and fill out your health history information as well as interact with your health care team. Filling out your health history and personal demographics will streamline your first visit with us, giving more time and attention between you and your healthcare provider.
